All individuals traveling to the Dominican Republic are required to complete an online entry form, available to be filled out approximately 72 hours prior to departure. The form can be completed on any electronic device with internet access. You can locate and complete the form below. Once you have successfully filled out the form, you must present proof of completion (paper copy or on a mobile device) upon arrival in the Dominican Republic. Individuals traveling from the Dominican Republic are also required to complete an online exit form upon departure from the Dominican Republic. Internet service at the airports can be slow so we recommend completing the departure form in Dominican Republic before leaving the hotel.
Casa de Campo Resort & Villas has set up a Testing Lounge to provide both Antigen tests and PCR test for guests returning to their countries of origin. The cost per test is $45.00 for the antigen tests and US$72.00 for the PCR test. Appointments can be scheduled by emailing vacationplanner@ccampo.com.do.
